A REVOLUTION IN SMALLGROUP BENEFITS. A new product co-developed by Blue Cross and Groupguru Inc. EVOLUTION© is a comprehensive program especially created for small businesses. This group insurance product does more than meet your needs, it anticipates them! We offer the flexibility your client's needs, while providing employees from 3 to 19 lives the best insurance available in the market place, as well as non-medically underwritten. The component driven program offers a full range of benefits, such as:

• Component 1: Life Insurance, Accidental Death & Dismemberment.
• Component 2: Drugs, Health Benefits, Medical Devices, Semi-Private Hospital, Private Duty Nursing.
• Component 3: Dental benefits including basic, preventative, endodontics and periodontics.

OPTIONAL BENEFITS

• Option 1: Dependent Life.

  Optional Life Insurance, Optional Accidental & Dismemberment (this
  benefit is subject to review and pricing by Ontario Blue Cross
  Underwriting).

• Option 2: Long Term Disability (minimum 5 employees) this benefit is subject to review and pricing by Ontario Blue Cross underwriting.

Each component will also offer different "levels" of coverage. They are defined as Bronze, Silver, Gold and Platinum. Each company will be able to select from each component the level of coverage that best suites their needs and budget. They will be able to "mix and match" coverage by selecting whichever component they want.
